FL Wind Mitigation Form OIR-B1-1802 (Rev. 04/26) – Photo Sections Missing & Labeling Issues

Two issues with the current ISN implementation:
- MISSING PHOTO SECTIONS FOR Q2 AND Q3 – Questions 2 (Region / wind-speed design region) and 3 (Roof Slope) are new to the revised form and currently have no dedicated photo upload/placement section in ISN. Both require at least one photo per the form's requirements. These need a photo slot just like the existing Questions 4–9 already have.
- PHOTO LABELING NEEDS TO REFERENCE WHAT IS BEING PROVEN – Each photo across all questions (2–9) should carry a label that corresponds to the specific attribute the photo is intended to validate. For example:
• Q2 – "Wind-speed design region per ASCE 7-22"
• Q3 – "Roof slope verification (≥ 6:12 or < 6:12)"
• Q4 – "Roof covering type / product approval"
• Q5 – "Roof deck attachment – nail size and spacing at adjacent trusses"
• Q6 – "Roof to wall connection – connector type and nail count"
• Q7 – "Roof geometry – [North/South/East/West elevation]"
• Q8 – "SWR – underlayment type / installation verification"
• Q9 – "Opening protection – [North/South/East/West elevation] – product approval or coverage type"
Generic labels like "Photo 1, Photo 2" don't help the carrier reviewer tie the image back to the attribute it's supposed to prove. Descriptive, question-specific labels reduce rejection risk and speed up carrier processing.
These are compliance issues. Carriers are already enforcing the new form requirements, and reports that don't clearly tie labeled photos to each question risk rejection.
Thank you.
